Diet shaped gut microbiota more strongly than PCOS pathology, yet PCOS selectively reduced beneficial Bacteroides acidifaciens. FMT shifted microbial profiles toward normal but did not reverse metabolic or reproductive traits, highlighting diet-microbe interactions as potential therapeutic levers.
